THE

COMMUNITY LIVING

OPTION

 

 “Why can’t I live at home?”

 

“Where can I find the support and services to live more independently?”

 

 If you or someone you know who is living in a nursing home has asked these questions, then we may have the program for you.

 

 

 

You may have the option to live in the community!

 

Many individuals may have initially been placed in a nursing home because of a medical condition.  This initial placement was considered a medical necessity.  However, once medically stable, people may continue to reside in an institutional setting only due to lack of knowledge of the Community Living Option  

 People who are receiving long-term care services in nursing homes have more options to live independently in the community than ever before.  However, the decision to move and the task of lining up necessary community-based services and supports can be complicated and confusing. An agency that is familiar with community resources and programs may be able to facilitate this transition.  The Ocean State Center for Independent Living (OSCIL) should be the agency you call.

Since 1998 the Ocean State Center for Independent Living’s Community Living Option program has provided assistance to persons with disabilities to move from the nursing home setting to a less restrictive environment.  OSCIL’s knowledgeable staff works with each person to:

 

·     Establish community connections

·     Find accessible housing

·     Coordinate community support services

·     Access public transportation services

·     Acquire appropriate assistive devices

·     Secure home safety devices

·     Assist with accessing appropriate home adaptations

 

Essentially, OSCIL will assist with any service that may be required to facilitate a safe transition back to the community.

 

What are home adaptations?

 

Home adaptations are any change made to the home that will increase access for a person with a disability. Examples include installing ramps, widening doorways or installing safety rails.

 

What are assistive devices?

 

Equipment such as reachers, tub benches, alerting systems and other devices that can help you perform tasks in your home.

 

What are home and community support services?

 

Any service(s) that may be necessary to stay in the home.

Services may include:

·        Home Health Agency  Homemakers, Aides, LPNs, Nurses and Therapists

·        Assisted Living Services

·        Meals on Wheels

·        Personal Emergency Response Systems

·        Adult Day Care

 

The Community Living Option program serves people with disabilities who are capable of living more independent lives.  General eligibility requirements include:

·        The presence of a significant disability

·        Motivation to actively participate in the program

·        Indication that the individual will benefit from participation in services.

 

Numerous studies around the country are showing that people with even very significant disabilities are able to live safely and comfortably in their own homes or apartments when provided with the right supports. Further, many people report being happier and healthier when they have their independence and own home environment.
